You need to register yourself on Matrix website using your Matrix Mobile Number & Customer Agreement Form Number (this is mentioned on the to right of the form). We will send you the User name & Password to the registered email ID. You can then simply click on View My Bill tab on Matrix website, log in using the User name and Password and view your bills.
